Our Founder & Director of Operations has over 20 years of experience in healthcare. Transformation Continuum Health was founded with the purpose and vision to deliver superior homecare services to meet the total needs of individuals beyond the routine office or hospital setting. As a Doctorate level Family Nurse Practitioner, our founder has seen firsthand the barriers individuals face daily while striving for independence and maintaining quality of life.
The continual witnessed difficulties and failures experienced with meeting basic daily needs in the home prompted the pursuit of purpose and passion to provide meaningful care in the home. For this reason, our founder is dedicated to overseeing all aspects of business operations, ensuring that at Transformation Continuum Health, service standards are continuously met, and our team remains at the forefront of industry best practices.
